Help provide oral health education books to schools that the Tooth Taxi visits by sponsoring a Tooth Taxi Library Pack today. Your tax deductible donation of $50 sponsors a pack of four books that provide invaluable oral health information for teachers, parents and children. A sticker indicating your donation will be placed in each book and given to schools by Tooth Taxi staff as they travel around the state giving free dental care and oral health education to students throughout Oregon.

There are three ways to order your Tooth Taxi Library Pack:

1. Go to ONLINE DONATIONS on this website and in the comments section simply
   write that you want your donation to support a Tooth Taxi Library Pack.
2. Fill out the Order Form (HERE), then fax it to our office at 503-218-2004
   or mail it to DFO, PO Box 2448, Wilsonville, Oregon 97070.
3. Call the DFO office at 503-594-0880 to order by phone.
